Paciente Seguro is a nationwide collaborative project (2016–2023) that advanced patient safety in Brazil through innovative educational strategies. Led by Hospital Moinhos de Vento in partnership with the Brazilian Ministry of Health under a national development initiative, this project implemented the National Patient Safety Program across 96 public hospitals in all regions. A multifaceted educational approach was used, including 25 hours of online courses with over 23,000 enrollments, the distribution of nearly three million printed and digital educational materials, and engaging physical and digital games to reinforce safety concepts. The project also invested in institutional capacity-building by training multidisciplinary hospital teams and establishing functional patient safety committees at each site. Strategic methodologies were employed to drive improvement. The Model for Improvement framework guided the implementation of six national patient safety protocols—covering patient identification, medication safety, surgical safety, hand hygiene, fall prevention, and pressure injury prevention. Continuous monitoring and support were provided through regular virtual check-ins, on-site consultations, and collaborative problem-solving diagnostics among participating hospitals. As a result, the initiative achieved significant reductions in adverse events and improved compliance with safety practices. For example, participating hospitals saw substantial decreases in patient falls and pressure injuries, alongside marked increases in hand hygiene adherence and surgical safety checklist use. These improvements translated into an estimated cost savings of approximately R$ 9 million per year for the public healthcare system. Equally important, the project fostered greater engagement of healthcare professionals and patients in safety efforts, instilling a culture of safety on the front lines of care. In the long term, Paciente Seguro aims to drive a lasting cultural transformation in healthcare, empower patients as partners in their own care, and disseminate best practices in patient safety throughout Brazil’s healthcare system.
